Scope:
This policy applies to students. Distance education students no physically present on campus are exempt from this policy.
Policy Content
- Students are required to complete a self-reported Health History Form in the Patient Portal prior to matriculation.
- Tuberculosis Screening must be performed within the six months prior to matriculation. Students are required to complete Tuberculosis Risk Screening. Based on responses students are stratified into low-risk and high-risk categories. High risk students are required to complete the “Tuberculosis Clinical Evaluation and Testing” form. This applies to all students living on campus, even those claiming exemption to the immunization requirement.
- The State of Texas requires college students under 22 years old to have the Meningococcal Conjugate Vaccine. This vaccine protects against four serogroups A, C, W, and Y. It is referred to as the MCV4 Meningococcal vaccine. This vaccine must be administered within 5 years of the start of classes and at least 10 days before classes start. If you have questions about this requirement, please refer to the “Texas Minimum State Vaccine Requirements…” on the Texas Department of State Health Services website. There is a process for claiming exclusion from the Meningococcal Conjugate Vaccine requirement. That “affidavit” process is outlined on the state website
All students, undergraduate or graduate, enrolling for nine or more hours or participating in a full-time graduate program are required to have health insurance. To ensure compliance with this requirement all students will be billed for the Student Health Insurance Plan, SHIP, annually. This policy also applies to those students returning from leave or matriculating in the Spring semester. All students will be enrolled in the SHIP at the end of Add/Drop unless the online insurance waiver, providing proof of personal health insurance, has been approved. The deadline for submitting an insurance waiver is the end of Add/Drop, approximately six days after classes start. Students are not eligible for a refund of the SHIP premium if the online waiver, with proof of insurance, has not been approved by the end of Add/Drop, which is six class days or eight days after classes begin. For students who request an accommodation to the insurance policy on a religious basis please refer to the Student Religious Accommodation Request Policy.
New students or those returning from leave are notified of Health Record and Immunization Record deadlines by the Office of Admissions and Academic Advising.
Students requesting waiver of the requirement for SHIP based on religious objection must do so by following the Student Religious Accommodation Request Policy. This must be completed annually before the fall semester and when returning from leave. The deadline for requesting waiver is the end of Add/Drop, which is six class days or eight days after classes begin.

A student, who has not provided the required documentation of the Meningococcal vaccine to Health Services, as required by Texas law, will not be permitted to attend classes until the requirement is met. The law allows the University to permit a 10-day extension of the deadline in special circumstances, e.g. a student who lives in an area where the Meningococcal vaccine is not available. A request for a 10-day extension does not release the student from the requirement and must be submitted to Health Services for consideration.
Students who fail to provide the Health History and Tuberculosis Screening requirements jeopardize their continued enrollment at the university. A restrictive hold will be placed on that student's account preventing registration for the following semester until the required documentation has been provided.
